A practitioner guide has been developed by the UK’s National Institute for Health and Care Excellence (NICE) and the Social Care Institute for Excellence (SCIE) to help those supporting young people transition to adult services. The Building independence through planning for transition quick guide explains how early planning and access to the right information can help young people and their families to make the right choices. The guide covers how practitioners can ensure that enough information is available, draw up transition plans, and topics to consider such as managing long term conditions. There is also a section for further resources and information. The guide is relevant to all practitioners in children’s and adults’ services who support young people at transition. An accompanying video is also available where a young person talks about his experience of transition.
